Tire Information (All Vehicles and Trailers)

All information is found on the tires. The information may not be in the exact location found in the image. The Age Code should be read and entered from each tire. The Age Code is usually listed on one side of the tire, which may require you to find it on the other side. Verify the information on each tire, as sometimes the tires are different. The tire pressure should be listed at what you usually keep the tires. If you don’t know, check each tire’s tire pressure and write that on the form for each tire.

Motorhome/Van/Bus Information

You can use the diagram to fill out the information from the placard. If you have a tag axle, the tag information will be on the placard, similar to the REAR GAWR.

Truck/Tow Vehicle Information

Use the diagram to fill out the information from the placard.
